A Special Message for City of Chicago Employees

Dear City of Chicago Employees,

Your benefits enrollment period is October 25, 2004 – November 12, 2004. Choosing your health benefits plan is a very important decision, for both you and your family. That is why it is important to know that when choosing UniCare, you are choosing with confidence.

UniCare HMO Performance offers comprehensive benefits while allowing access to a large network of quality providers. This network of independently contracted* providers includes:

  • 2,000 Primary Care Physicians
  • 4,000 Specialists
  • More than 90 hospitals in the metropolitan Chicago area
  • Advocate Health Centers
  • Access to over 1,200 pharmacies, including most major chains

Please note: If you are a current UniCare HMO member and your primary care physician participates in the UniCare HMO Performance network, you will not have to re-enroll. You will be moved over automatically.

With UniCare HMO Performance you will also experience customer service committed to serving you better, as well as access to Preventive Health Programs and Health Improvement Programs.

To receive provider directories, please see your benefits liaison. Or, attend one of the City’s Benefits Information Fairs on the following dates:

  • Wednesday, November 3rd - 10:30 a.m.- 2:30 p.m. Police Headquarters, 3510 S. Michigan Ave. (1st Floor Multi-purpose Room)
  • Thursday, November 4th – 11:00 a.m.- 4:30 p.m. O’Hare Airport, Terminal One (OCC - Office of Communication Center)
  • Tuesday, November 9th – 11:30 a.m.- 2:00 p.m. and 5:00 p.m. - 7:00 p.m. DePaul Center, 333 S. State St., 4th Floor, Conference Room A & B
  • Wednesday, November 10th – 11:30 a.m.- 2:00 p.m. City Hall, 121 N. LaSalle St., Room 1103
